The forcibly pulled and sutured skin concentrates a strong 'Tension' that tries to pull apart in both directions. If the skin cannot withstand this tight tension, the surgical site can tear, causing the scar to widen more severely than before, or in the worst-case scenario, blood circulation may be blocked, leading to fatal tissue necrosis.

After waiting several months for the scar tissue to soften and the skin to stretch again to create new excess tissue, the 2nd surgery is performed. During the 2nd revision, the remaining depressive scar is completely removed, while the caruncle exposure and shape distortion that occurred in the 1st surgery are refined into their natural V-shape through double flap techniques and precise multi-layer suturing to complete the final eye shape.
